LeDeR: Learning from Lives and Deaths of People with Learning Disabilities and Autistic People

The LeDeR (Learning disabilities mortality review) Programme reviews the lives and deaths of people with learning disabilities and autistic people across England. The aim of the programme is to learn from people’s experiences of using health and social care services in Surrey and make changes to make sure that people with learning disabilities and autistic people receive high quality care which helps them to stay healthy and get the support they need. How to Report to LeDeR

Anyone can notify the LeDeR programme about someone who has died.

If you need help with this, please call 0300 561 1606 (Monday to Friday, 9am to 5pm) or 01278 727411 (Monday to Friday, 8.30am to 4.30pm) to talk to someone who can help you. Please note these numbers will not be available on bank holidays.

National and Surrey LeDeR reports

National Report:

This annual report from the University of Bristol provides information about the deaths of people with a learning disability aged four years and over notified to the programme. There is also an easy read version available.

Surrey Heartlands Annual Report

The annual report page gives an overview of the LeDeR programme and how this has been implemented across Surrey Heartlands, including reports for previous years. It provides an overview of the number of deaths that have been reported to LeDeR throughout this period and summaries the learning that has come from the completed reviews.

LeDeR Strategy: Putting the learning from LeDeR into Action

Surrey developed an Action From Learning Report 2020 to 2021 in response to the learning from LeDeR reviews.

There is now a Surrey Heartlands Strategy 2021 to 2024 that sets out what we would like to achieve over the next 3 years to make care better for people with learning disabilities and Autistic people in Surrey.
